OPEN POSITION: Rapid Rehousing Case Manager

Job Description: Case Manager – Rapid Rehousing Program Manager
Summary:
This position is responsible for providing client-centered case management services involving a great deal of direct service interaction, partnering with various stakeholders, and system collaboration for our Rapid Rehousing program.
Essential Duties and Responsibilities: Case Management
General
-
Document work in case files (and other methodologies as appropriate, including but not
limited to HMIS, spreadsheets, etc.) according to agency protocol, to meet or exceed
requirements set by grantors and professional ethics guidelines.

Document all client contact, including case management meetings, emails, and phone
conversations.

Create mutually agreed upon goals and track/document via goal sheets helping clients
accomplish and work towards self-sufficiency.

Complete monthly budgets with clients and work with clients to adhere to the determined
budget. Rapid Rehousing
-
Complete required NC DHHS Emergency Solutions Grant (ESG) timesheet/billing forms.
-
Manage a caseload of 20-25 RRH households.
-
Build professional relationships with clients, providers, and community partners.
-
Landlord engagement: (1) Recruiting, building rapport and managing retention of
landlords/property managers, (2) Quarterly check-in with established landlords/property
managers.

Work with community Housing Navigation Unit as necessary to move clients into
permanent housing.

Housing Search & Placement: (1) Meet with clients minimum 2 scheduled times per
month, (2) Work with the client through consumer choice to determine best options for
permanent housing.

Housing Stabilization: Complete monthly home visits to support families in performing
the tasks required to maintain housing stability, including on-time payment of rent and
household maintenance

Submit financial assistance payment requests to the Program Director in a timely manner.
All Direct Care Staff Responsibilities

Participate in mandatory on call rotation for support to congregations, Day Center volunteers, and residents.

Participate in rotation of Monday evening shifts to provide Day Center coverage and assist volunteers.

Communicate with host congregations through van sheets, host info, family updates, etc.

Act as task supervisor to a minimum of one intern.

Develop and maintain assigned strategic external partnerships and represent agency at
relevant community meetings and events.

Verify proper and complete data collection on program outputs and outcomes; maintain
expertise in NC HMIS use and reporting.

Attend staff, Board committee, and community meetings as directed. Participate in staff
presentations and/or represent the agency in the community when needed. Cooperate with volunteer
involvement, work teams, and donation drives.

Qualifications:
A bachelor's degree in a human service field or comparable work/life experience required. One year of experience working with populations experiencing homelessness, specifically in Rapid Rehousing/housing first model, required. Knowledge of Wake County service delivery system preferred. Successful experience in human services program delivery and management is primary qualification. Strong assessment skills, case management experience, and the ability to function as part of a team preferred. Proficiency with MS Office tools (Word, Excel) and Google Suite (Gmail, Calendar, Drive) is essential. Ease in data entry in the required internet-based database required. Fluency or facility in Spanish is a plus.
Working Hours and Conditions

This position is grant-funded and is subject to renewal. Currently funded for minimum of 1 year.

Full-time: Tuesday through Saturday
-
Required participation in the 24-hour on-call rotation including nights, weekends, and
holidays; 2 weeks on with 6 weeks off.

Must be willing to work a flexible schedule, including holidays.

Position requires a combination of office-based and community-based duties for
meeting/events and service delivery.

Must conduct client home visits.
